1. Fill out all 3 pages of form "NEW PATIENT FORM"

2. If we need old records, fill out "RECORDS RELEASE"

3. Have your DENTAL insurance card in hand upon arrival.

4. Be aware we do NOT submit to Medicaid OR Medicare.  We DO submit to all other dental insurances and are "In-Network" with Total-Cigna and Delta Dental Premiere.
